When should I increase the weight for my exercises? How much should I add? Should I lower or increase the repetition range?

It is a good rule of thumb to increase weight around 10-15% after you have increased reps from 10 or 12 to a maximum of 15. Once you have increased the weight, drop the reps back down to the 10-12 range. Repeat this cycle.
